The Clearinghouse Applicant Initiated (CHAI) website is for individuals screening for Florida Department of Health (DOH) licensure purposes. Using the CHAI website for screening purposes outside of Florida Department of Health licensure is an FBI Criminal Justice Information System Security Policy violation. If you are not a DOH licensee, please contact your employer for further background screening instructions.
Attention
Effective October 1, 2026, the FBI will be increasing its fingerprinting fee from $12 to $15. The cost of all screenings except Agency Reviews will be increased due to this change. Florida Department of Law Enforcement has contacted Livescan vendors to inform them of this change. If you have any questions, please contact FDLE.
